Abstract Title:Combining Sleeve Gastrectomy and Hiatal Hernia Repair for GERD Management

Research Interest:

Sleeve gastrectomy (SG) is currently the most widely performed bariatric procedure worldwide, offering a balance of safety, simplicity, and effective weight loss. However, concerns have emerged regarding the increased incidence of postoperative gastroesophageal reflux disease (GERD), with some patients developing de novo or worsening hiatal hernias (HH) following SG. These conditions can lead to significant symptoms and may require revisional surgery. Clinical observations suggest that many patients who develop severe GERD or sliding HH after SG may have had subtle or underappreciated hernia findings on preoperative endoscopy, indicating the importance of thorough preoperative assessment and appropriate surgical planning. This study aims to investigate whether performing hiatal hernia repair (HHR) during SG can reduce the risk of postoperative complications such as severe GERD and progressive HH. In our recent practice, we adopted a procedural method where HHR was performed at the time of SG for all patients with HH confirmed on preoperative endoscopic evaluation. This consistent surgical approach allows us to evaluate the technical feasibility and potential benefits of combining HHR with SG in selected patients.
